Enrolling is simple:
- Download, complete and submit (HIPAA Compliant Patient Privacy Email) your Consortium Agreement along with a list of drivers on a spread sheet or separate document. You can also fax the documents to 318.855.8453. (*Please note: For FMCSA drivers include Driver’s License Numbers. All others include SS#’s.)
- We will contact you with our affordable consortium pricing and confirmation of enrollment. You are welcome to contact us at anytime.
